The story follows Manuela (Cecilia Roth), a single mother whose son is killed in a car accident on his 17th birthday. Overcome with grief, she leaves Madrid for Barcelona, looking for her son's long-lost father. The father, wouldn't you know, is now transgendered, and goes by the name of Lola. While searching for Lola, Manuela meets up with an old friend, Agrado (Antonia San Juan) at the place where transvestite prostitutes ply their trade and is taken on as personal assistant to the famous stage actress, Huma Rojo (Marisa Paredes), who was admired by Manuela's son before he died. She lives with Nina (Candela Pena), a tempestuous drug addict, who plays Stella in a touring production of A Streetcar Named Desire. She becomes involved with Sister Rosa (Penelope Cruz), a young volunteer who dreams of working with the poor in El Salvador but is pregnant by Lola and now HIV positive. Their journeys are individual but also ingeniously connected, with Manuela serving as both the compass and the foundation for their intertwining lives.

Almodòvar ends the film with this dedication: "To all actresses who have played actresses. To all women who act. To men who act and become women. To all the people who want to be mothers. To my mother" Although dealing with tragic situations, Almodovar blends in the humour with the ease and efficiency of a master craftsman.
